What is DA Form 3078?

DA Form 3078, Personal Clothing Request, is used when personal clothing items are requested, issued, or exchanged for a soldier. The form lists the clothing items and quantities line by line and records the authorization for the transaction, creating an accountable record of what the soldier received.

The current edition on Army Publishing Directorate is dated March 2025, with the Army G-4 as proponent. Personal clothing policy, including who is entitled to issues and how transactions are authorized, is covered by AR 700-84.

Who files DA Form 3078?

Soldiers requesting initial or replacement personal clothing items
Clothing issue point (CIF or CIIP) staff documenting issues and exchanges
Unit supply personnel preparing clothing requests for their soldiers
Approving officials authorizing clothing transactions

How to fill out DA Form 3078

Enter the soldier's name, rank, SSN or DoD ID information, and unit as the form requires.
List each clothing item requested with its size and quantity on the item lines.
Mark the reason for the request, such as initial issue, replacement, or exchange.
Obtain the authorizing official's signature for the transaction.
Take the approved form to the clothing issue point, where the issue is completed and receipted.

Who pays for replacement clothing on DA Form 3078?

It depends on the reason for replacement. Items lost or damaged through no fault of the soldier are handled differently from fair wear and tear or soldier-caused loss. AR 700-84 and your supply office determine whether the issue is gratuitous or paid.

Is DA Form 3078 the same as a CIF hand receipt?

No. Organizational clothing and individual equipment (OCIE) from the central issue facility is receipted separately. DA Form 3078 covers personal clothing, the uniform items that belong to the soldier.
