DA Form 5500, Army Body Composition Screening and Assessment Worksheet, records the height and weight screening required by the Army Body Composition Program and the follow-on body fat assessment for Soldiers who exceed the screening weight for their height and age. The current program under AR 600-9 uses a circumference-based tape assessment to estimate body fat.
The worksheet captures the Soldier's measurements, the calculated body fat percentage, and whether the Soldier meets the standard. Trained unit personnel take the measurements, and the completed form supports enrollment in or release from the Army Body Composition Program.
