DD Form 626, Motor Vehicle Inspection (Transporting Hazardous Materials), is the checklist used to inspect trucks and drivers before hazardous materials are loaded and before they leave a Department of Defense installation. Inspectors work through items covering the driver’s license and credentials, shipping papers and emergency response information, and the vehicle’s condition: brakes, tires, lights, fire extinguishers, placards, and cargo securement.
