What is EP/S Pass Candidate Form?

Employment Pass and S Pass applications are made by the employer or an appointed employment agent through MOM's EP eService, not by the candidate. The candidate's form exists so the employer can send the candidate a single document to complete, then use those answers when filling in the online application.

The form covers the candidate's name as shown in the passport, date and place of birth, nationality, passport details, residential address, marital status and dependants, education and professional qualifications, employment history and the job being offered. It also carries the candidate's declaration and consent for MOM to verify the information given.

How to fill out EP/S Pass Candidate Form

Do I send this form to MOM myself?

No. The candidate's form goes back to the employer or the appointed employment agent. They submit the actual Employment Pass or S Pass application online through EP eService.

What happens after I return the form?

The employer uploads the required documents and submits the application. MOM says most standard cases are processed or updated within about ten business days, with longer timelines for overseas companies without a Singapore registered entity.

What documents will be needed?

Typically the passport personal particulars page, educational certificates and, depending on the case, employment references or professional registrations. Check the documents required list for your pass type on mom.gov.sg.
