What is G-1450?

Form G-1450 authorizes USCIS to charge a filing fee, and the biometric services fee where one applies, to a credit card, debit card, or eligible prepaid card. It is used with paper filings sent to locations that accept card payment, such as USCIS lockbox facilities.

You complete one authorization with the cardholder's information, the card details, and the amount, then place it on top of the application it pays for. USCIS charges the card once; if the payment is declined, USCIS rejects the filing rather than retrying the charge, so the card details and available balance need to be right.

How to fill out G-1450

Check the current fee for your form at uscis.gov/fees and confirm the filing location accepts Form G-1450.
Fill in the cardholder's name, billing address, card type, number, expiration date, and the exact amount to charge.
Have the cardholder sign the authorization.
Place the completed G-1450 on top of your application or petition package.
Mail the package to the address listed for your form; USCIS destroys the card information after processing the payment.

What happens if my card is declined?

USCIS will reject the entire filing and return it to you. It will not hold the case while you fix the payment, so a rejected charge can cost you your place in line or a deadline. Verify the card number, expiration date, and available credit before mailing.

Can I use one G-1450 for several applications?

Follow the current form instructions on this point. In general, each filing should have its own payment authorization so a problem with one case does not affect the others.
