What is Form 6?
A Singapore permanent resident who travels abroad without a valid Re-Entry Permit loses permanent resident status. Form 6 is the paper application for that permit, made under section 55(1) of the Immigration Act 1959 and the Immigration Regulations.
The single page form collects the applicant's full name in block letters with the surname underlined, name in Chinese, Jawi or Tamil characters where applicable, NRIC or UIN, alias, date of birth, sex, nationality, marital status, occupation, gross monthly salary, residential address and contact number, travel document details including issue and expiry dates, and the highest academic or professional qualification attained. Part II is a declaration that the information is true and consent for ICA to verify it. Where the applicant is under 16, a parent or legal guardian signs.
Who files Form 6?
- Singapore permanent residents applying for or renewing a Re-Entry Permit
- Permanent residents whose current permit is close to expiry and who plan to travel or remain abroad
- Parents and legal guardians applying on behalf of a permanent resident child under 16
- Applicants directed by ICA to submit the printed form rather than the online e-Service
How to fill out Form 6
- Print the form and complete Part I with your full name in block letters, underlining the surname, and add characters in Chinese, Jawi or Tamil if applicable.
- Enter your Singapore NRIC number or UIN, date of birth, nationality, marital status, occupation, gross monthly salary and residential address.
- Complete the travel document section: type, number, country or place of issue, and the issue and expiry dates.
- Read the Part II declaration, then sign and date it. For an applicant under 16, the parent or legal guardian signs and gives their full name.
- Submit the completed form to ICA as directed for your case. Renewal and transfer of a Re-Entry Permit are normally done through the ICA e-Service, which is the faster route.
