Form 12: Dependant's Pass / Long-Term Visit Pass Application Form
Form 12 is the Ministry of Manpower application for a Dependant's Pass or Long-Term Visit Pass for family members of Employment Pass (Sponsorship) candidates and holders.
A Dependant's Pass lets the spouse and unmarried children under 21 of an eligible pass holder live in Singapore, while a Long-Term Visit Pass covers other family members such as parents and common law spouses. Most EP and S Pass employers apply through the EP eService on the myMOM Portal, but dependants of Employment Pass (Sponsorship) candidates and holders have to use this 10 page paper form instead.
Parts 1 to 6 are completed by the foreigner and the work pass candidate or holder and cover the application information, the foreigner's personal particulars and travel document, vaccination requirements for foreign born children aged 12 and below, immigration history questions, and signed declarations. Parts 7 and 8 are completed by the local sponsor and, where used, the employment agency. MOM asks that the form be opened and completed in Adobe Acrobat Reader so it displays correctly.

Local sponsor companies and employment agencies submitting the application on the pass holder's behalf
How to fill out Form 12
Download the form and open it in Adobe Acrobat Reader; the foreigner and work pass candidate or holder complete Parts 1 to 6 on pages 2 to 6.
Fill in every field, entering Not applicable or N.A. rather than leaving anything blank, and answer the immigration history questions in Part 4 in full.
Sign the declarations in Parts 5 and 6; a parent signs for a foreigner under 16.
Send the form to the local sponsor, who checks the earlier parts and completes the sponsor declaration in Part 7, plus Part 8 if an employment agency was used.
The local sponsor uploads the signed form and supporting documents at go.gov.sg/submit-dependants-of-ep-sponsorship using Corppass and pays the application fee; processing is within 6 weeks.
Worth knowing: The local sponsor's organisation needs a Corppass account to upload the form and pay. Complete the PDF in Adobe Acrobat Reader, since other readers may not display the fields correctly.

Can I use this form if the main pass holder has a normal Employment Pass or S Pass?
No. Form 12 is only for dependants of Employment Pass (Sponsorship) candidates and holders. For dependants of standard EP or S Pass holders, the employer submits through the EP eService at go.gov.sg/ep-eservice.
What supporting documents are needed?
A copy of the travel document page showing the dependant's particulars, plus the marriage certificate for a spouse or the official birth certificate for a child. Children who were not born in Singapore and are aged 12 and below also need the Verification of Vaccination Requirements document from the Communicable Diseases Agency. Non-English documents need an English translation.
How much does it cost and how long does it take?
There is a fee on submission and a further fee when the pass is issued, payable by PayNow, Visa, Mastercard or Amex. Check the current amounts on mom.gov.sg. Processing is within 6 weeks, and you can track the case at go.gov.sg/check-work-pass-application after a week.
