What is Form V75?
A visitor whose treatment or recovery runs past the period of stay granted can ask ICA to extend that stay on medical grounds. Form V75 is the supporting application, addressed to the Controller of Immigration at the ICA Services Centre.
Section A is completed by the doctor, who names the patient and passport number, states the number of extra days sought, and gives a detailed description of the medical condition, the treatment plan and frequency of treatment, the next appointment date, whether the patient is fit to travel and whether travel would be life threatening. The doctor signs with their MCR number and the hospital or clinic stamp. Section B is the patient's declaration consenting to disclosure of confidential medical information, including diagnosis, to ICA.

How to fill out Form V75
- Ask your treating doctor to complete Section A with your name, passport number and the number of additional days requested.
- The doctor sets out the medical condition, the treatment plan and frequency, the next appointment date, and whether you are fit to travel or whether travel would be life threatening.
- The doctor signs and dates Section A with their MCR number, telephone number and the hospital or clinic stamp and address.
- Complete Section B yourself, signing the declaration that consents to the disclosure of confidential medical information, including diagnosis, to ICA.
- Submit the form to the Controller of Immigration, Immigration and Checkpoints Authority, ICA Services Centre, 2 Crawford Street, Singapore 207218, following any submission instructions ICA gives for your case.

Do I still need to apply for the extension itself?
Yes. Extensions of a Visit Pass are applied for through the ICA e-Service and are subject to approval. Form V75 is the medical evidence that supports an extension sought on medical grounds.
Who has to fill in the medical section?
The treating doctor. Section A asks for the diagnosis, treatment plan, next appointment and a fitness to travel assessment, and it must be signed with the doctor's MCR number and the clinic or hospital stamp.
What happens if I overstay while waiting?
Overstaying is a serious offence in Singapore. Apply before your current pass expires and keep evidence of the application. The form also warns that making a false statement, representation or declaration is an offence under the Immigration Act.
