What is Formulier 1?
An eenmanszaak has exactly one owner, even when other people work in the business, and that owner can only ever have one of them. It can, however, run several branches. Form 1 puts the business into the Handelsregister: its trade name, what it does, its addresses and the owner details KVK needs to identify you.
The form covers two situations: registering a brand new eenmanszaak where the owner does not have to attend a KVK office in person, and reporting that you are taking over an existing business or branch and continuing it as an eenmanszaak. In every other case KVK asks you to register online instead, at kvk.nl/inschrijveneenmanszaak.
Who files Formulier 1?
- Anyone starting a Dutch sole trader who cannot or does not want to use the online registration
- Someone taking over an existing business and continuing it as an eenmanszaak
- Owners registering before trading starts, for example to be able to request quotes
How to fill out Formulier 1
- Say whether this is a new eenmanszaak or a continuation, and give the start date in dd-mm-yyyy. A continuation needs two dates: the takeover date from the contract, and the original registration date of the business.
- Enter the trade name and describe the activities in your own words. What you write here becomes the public description of the business.
- Give the visiting address and, if it differs, the postal address, plus the phone number, email address and website.
- Fill in the owner details, including the burgerservicenummer, and name a valid ID document: a passport, Dutch driving licence, European identity card or Dutch residence document.
- Print the form, sign it with a pen, and follow kvk.nl/formulier1 to see whether to post it or bring it to an appointment.

Can I sign this in the browser and email it?
No. KVK asks for a signature written with a pen and will not accept a copy or a scan of one. Fill the form in here, print it, and sign the paper copy.
Can I have two eenmanszaken?
No. One person can own only one eenmanszaak. It can have several branches and several trade names, which is usually what people are actually after.
What if I make a mistake while filling it in on paper?
Cross out the wrong answer, write the right one next to it, and put your initials or signature in pen beside the correction. Do not use correction fluid.
