KVK form 14: change details of a business or branch
Form 14 changes what the Handelsregister says about a business or one of its branches, and it is also the form for closing or transferring an eenmanszaak or a branch.

What is Formulier 14?
The register has to match reality: the trade name, the activities, the visiting and postal addresses, the website, the phone number, the email address and the Berichtenbox name. Form 14 changes any of those for a business or a branch.
It also handles endings. Closing an eenmanszaak, closing a branch, or transferring either to someone else is reported here rather than on a separate deregistration form.
Who files Formulier 14?
- Businesses that have moved, renamed or changed what they do
- Owners closing an eenmanszaak or a branch
- Anyone transferring a business or branch to a new owner
- Businesses correcting an activity description that no longer fits
How to fill out Formulier 14
- Give the KVK number and, where the change concerns one location, the vestigingsnummer.
- Tick only the details you are changing and leave the rest blank.
- Write the new value beside each one, and give the date the change took effect as dd-mm-yyyy.
- For an ending or a transfer, give the date and say what happens to the business.
- Print the form, sign it in pen, and send it to KVK.

Frequently asked questions
How quickly do I have to report a change?
The Handelsregisterwet gives you one week from the change. In practice KVK processes late reports, but the obligation is a week.
Can I do this online instead?
Most changes can go through kvk.nl/wijzigen with DigiD or eHerkenning. The paper form is for the cases the online service does not cover, and for anyone without a Dutch login.
Does closing the business remove it from the register?
The entry stays visible with an end date. The Handelsregister is a historical record as well as a current one.
