What is Formulier 9?
A business can trade from more than one location, and each location is registered separately with its own address, activities and often its own trade name. Form 9 adds one of those branches to an existing registration.
It works for any business form: an eenmanszaak, a partnership such as a vof, cv or maatschap, or a legal entity such as a bv, nv, stichting or vereniging.
Who files Formulier 9?
- Businesses opening a second shop, workshop, office or site
- Owners who want a branch to trade under its own name
- A business taking over an existing branch and continuing it
How to fill out Formulier 9
- Give the KVK number of the business the branch belongs to.
- Say whether the branch is new or a continuation, with the date as dd-mm-yyyy.
- Enter the branch trade name and describe what happens at that location.
- Give the visiting address and, where it differs, the postal address and contact details.
- Print, sign in pen, and send the form to KVK.

Frequently asked questions
Does each branch get its own number?
Each branch gets a vestigingsnummer of its own. The KVK number stays the same for the whole business.
My branch has no visitors, it is a storage unit. Register it?
If work is carried on there for the business it is a branch and belongs in the register. Purely private storage is not.
