What is Formulier 10?
Partners are functionarissen: the register records who they are and what they may do on behalf of the partnership. A partner can be a person, another partnership, or a legal entity such as a bv.
For each partner the form asks for identifying details including the burgerservicenummer, and asks whether their authority is unlimited, limited or absent. A cv also distinguishes managing partners from silent ones.
Who files Formulier 10?
- Partnerships registering their partners for the first time, alongside form 2
- A vof or cv admitting a new partner
- A rederij registering its members
How to fill out Formulier 10
- Give the KVK number of the partnership, or file this with form 2 if it is a first registration.
- For each partner give the surname, first names, date of birth and burgerservicenummer, and their address.
- State the date the partner joined, as dd-mm-yyyy.
- Set out the authority: unlimited, limited with the limit described, or none.
- Print, have each partner sign in pen, and send the form to KVK with a copy of each ID document named.

Frequently asked questions
A partner has no burgerservicenummer. What then?
Leave the BSN blank. The form is explicit that a partner without one, typically someone living abroad, simply does not fill it in.
More partners than fit on the form?
Use a second copy of form 10. KVK processes them together.
