What is Formulier 16?
Officer covers a lot of ground: a partner in a vof, a managing partner in a cv, a maat, a member of a rederij, a sole shareholder, a director, a supervisory director or an authorised representative. Form 16 changes any of their registered details or ends the registration.
A resignation, a dismissal, a change of authority or a corrected date of birth all go here. The registered person and the entity are both identified, so KVK can match the change to the right entry.
Who files Formulier 16?
- Companies where a director has resigned or been dismissed

How to fill out Formulier 16
- Give the KVK number of the business or legal entity.
- Identify the officer whose registration is changing.
- Tick what is changing and give the new values, or say the registration ends.
- Give the effective date as dd-mm-yyyy.
- Print, sign in pen, and send the form to KVK.

Yes. A departing officer can report their own deregistration, which matters when the company is slow to do it and you want the public register to stop showing you.
