What is Formulier 11?
The Handelsregister records who may act for a bv, nv, cooperative, mutual, foundation or association, and with what authority. Form 11 is how those people are added, and it is one of the most used forms KVK publishes.
For each officer the form asks for the surname, first names, date of birth, burgerservicenummer and address, the role, the date of appointment, and whether authority is unlimited, limited or absent. An officer can also be another legal entity.
Who files Formulier 11?
- A newly registered bv or nv registering its board
- Companies appointing a new director or supervisory director
- A bv registering its sole shareholder
- Foundations and associations that run a business, registering their board
How to fill out Formulier 11
- Give the KVK number of the legal entity, or file this with the registration form for a new entity.
- Fill in the details of each officer, including the burgerservicenummer where they have one.
- Give the role and the date it started, written as dd-mm-yyyy.
- Say whether authority is unlimited, limited with the limit described, or absent.
- Print the form, have each officer sign in pen, and send it with a copy of each ID document named.

Frequently asked questions
Does a supervisory director have to be registered?
Yes. Commissarissen and members of a supervisory body are registered alongside directors, with their own start dates.
Can I do this online instead?
Changes to existing officers can usually go through kvk.nl/wijzigen. This form is the paper route and the one to use when the online service does not cover your situation.
What counts as limited authority?
Anything short of acting alone without a cap: jointly with another director, or up to a stated amount. Describe the limit exactly as the articles or the shareholder resolution puts it.
