What is Formulier 3?
A bv or nv comes into existence through a notarial deed, and the notary usually handles the first registration. Form 3 is the paper route: it records the company, its trade name, activities, addresses and share capital details.
It also covers a company that takes over an existing business or branch and continues it. The directors, supervisory directors and any sole shareholder go on form 11, and the ultimate beneficial owners on form 30.

- A newly incorporated bv or nv registering on paper rather than through the notary
- A bv or nv continuing a business it has taken over
- Companies whose notary has asked them to file the registration themselves
How to fill out Formulier 3
- State whether this is a new company or a continuation and give the date as dd-mm-yyyy.
- Enter the statutory name, the trade name if it differs, and describe the activities.
- Give the visiting address, postal address and contact details.
- Complete the questions about issued and paid-up capital.
- Print the form, sign it in pen, and file it with form 11 for the officers and form 30 for the UBOs.
