What is Formulier 13?
A gevolmachtigde acts on behalf of a business or legal entity under a power of attorney: signing contracts, placing orders, running a branch. Registering that authority means counterparties can check it in the public register.
A gevolmachtigd handelsagent is an independent agent who negotiates and concludes agreements for the business. One copy of the form registers one representative.
Who files Formulier 13?
- Businesses giving a manager or employee formal authority to sign
- Companies appointing a commercial agent who may conclude contracts
- Foreign companies giving a Dutch representative authority over their branch
How to fill out Formulier 13
- Give the KVK number of the business or legal entity granting the authority.
- Fill in the details of the representative: name, date of birth, burgerservicenummer and address, or the entity details if it is not a person.
- Give the start date of the authority as dd-mm-yyyy.
- Describe the authority: unlimited, or limited with the limit written out.
- Print, sign in pen, and send the form to KVK.

KVK registers what the form states. Keep the underlying power of attorney in your own records, and be sure the two describe the same authority.
