What is Formulier 6?
A foreign company that opens a branch in the Netherlands has to register that branch, along with the details of the company behind it: its legal form, its home register, and the people authorised to act for it.
The same form covers a foreign company with no Dutch branch that hires out staff in the Netherlands for payment. That is a registration obligation under the Waadi, the Dutch law on making labour available.
Who files Formulier 6?
- Foreign companies opening a branch, office or site in the Netherlands
- Foreign entities that supply staff to Dutch hirers for payment, with or without a branch
- Groups adding a Dutch establishment to an existing foreign structure
How to fill out Formulier 6
- Give the details of the foreign company: statutory name, legal form, country, and its registration number and register abroad.
- Enter the Dutch branch details: trade name, activities, visiting address and start date.
- Say who may act for the branch and what the limits on their authority are.
- Attach the supporting documents the form lists, such as an extract from the foreign register.
- Print, sign in pen, and send the package to KVK.

Frequently asked questions

Yes. Making labour available in the Netherlands for payment is itself a registration obligation under the Waadi, and this is the form for it.
Do the foreign directors have to be registered?
The people who may act for the Dutch branch are registered, and the form asks about the officers of the foreign company as well. Documents from the home register have to back this up.
