What is Formulier 15?
This is the entity-level counterpart to form 14. It covers changes to the statutory name, to the articles of association, to capital figures, and to the other details that belong to the bv, nv, vof, cv or maatschap itself rather than to the business it runs.
Changes made by notarial deed are usually filed by the notary. The paper form covers what the notary does not file, and situations where a correction has to be made directly.
Who files Formulier 15?
- Companies that have amended their articles
- A bv or nv changing its statutory name or seat
- Partnerships changing the terms recorded in the register
- A bv i.o. that has now been incorporated
How to fill out Formulier 15
- Give the KVK number of the legal entity or partnership.
- Tick only the details being changed.
- Enter the new values and the date each change took effect, as dd-mm-yyyy.
- Enclose the notarial deed where the change was made by one.
- Print, sign in pen, and send the form to KVK.

Trade name change, entity or business?
A statutory name belongs to the entity and goes on form 15. A trade name belongs to the business and goes on form 14.
