KVK form 18: supplement or amend another form
Form 18 is the continuation sheet for the Handelsregister forms. Use it when a form runs out of room, or to change something you entered on an earlier one.

What is Formulier 18?
Registration and change forms have a fixed number of blocks. When a partnership has more partners than fit, or when a limitation on authority needs more than the space allowed, form 18 carries the rest.
It is also the form for changing information already given on another form, including the details of a vereffenaar, and for the descriptions of limited authority that KVK asks you to write out in full.
Who files Formulier 18?
- Anyone whose registration form ran out of space
- Partnerships describing a partner limited authority in detail
- Filers correcting information sent on an earlier form
- Liquidators changing their registered details
How to fill out Formulier 18
- Give the KVK number, or say which form this supplements if there is no number yet.
- Name the form and the question the extra information belongs to.
- Write the additional or corrected information in full.
- Give the date it applies from, as dd-mm-yyyy.
- Print, sign in pen, and send it together with the form it belongs to.
