What is Formulier 21?
The Handelsregister shows who may act for an EESV. A manager can be a natural person, a partnership or a legal entity, and the register records the extent of their authority alongside their identity.
This form is filed with form 20 when the grouping is first registered, and on its own when a manager is added later.
Who files Formulier 21?
- An EESV registering its managers for the first time
- A grouping appointing an additional manager
- Members restructuring who runs the grouping
How to fill out Formulier 21
- Give the KVK number of the grouping, or file this with form 20.
- Fill in the details of each manager, including the burgerservicenummer where there is one.
- Give the start date of the appointment as dd-mm-yyyy.
- Describe the authority and any limit on it.
- Print, have each manager sign in pen, and send the form with a copy of the ID document named.
