What is Formulier 25?
Public law entities include municipalities, water authorities, joint arrangements between public bodies and other organisations created by or under public law. They appear in the Handelsregister like any other entity, so that anyone contracting with them can look them up.
The form records the name, the legal basis, the activities and the addresses. Officers of a public law entity that runs a business are registered on form 26.

How to fill out Formulier 25
- Give the name of the entity and the legal basis it was created under.
- Give the date it was established, as dd-mm-yyyy.
- Describe the tasks and activities.
- Enter the visiting address, postal address and contact details.
- Print, sign in pen, and send the form to KVK.
