What is Formulier 27?
A kerkelijke organisatie can be an umbrella body, a church organisation itself, or an independent part of one. All three can appear in the register, and the form covers each.
Registration is compulsory only in defined cases, which the form sets out on its first page. Outside those cases a church organisation may still register voluntarily, and many do because banks and counterparties ask for a KVK number.

How to fill out Formulier 27
- Give the name of the organisation and say whether it is an umbrella body, a church organisation or an independent part.
- Give the date it was established, as dd-mm-yyyy.
- Describe the activities.
- Enter the visiting address, postal address and contact details.
- Print, sign in pen, and send the form to KVK.

Is registration compulsory for us?
It depends on which of the cases on the first page of the form applies. Read those before filing, because voluntary registration has a different consequence for UBO reporting.
Do we have to register UBOs?
A church organisation can register UBOs on form 34 only where its registration is compulsory. A voluntary registration does not carry that obligation.
