What is Formulier 5?
A stichting pursues a purpose without members. A vereniging has members and a shared aim. A vereniging van eigenaars manages the shared parts of a building split into apartment rights. All three are legal entities and all three belong in the Handelsregister.
Form 5 registers the organisation itself. Its board members and any supervisory directors are registered on form 11 when the organisation runs a business, or on form 22 when it does not.

How to fill out Formulier 5
- Give the statutory name, any trade name, and the date the organisation was set up as dd-mm-yyyy.
- Describe the aim and the activities in your own words.
- Enter the visiting address, the postal address if it differs, and the contact details.
- Answer the questions about the deed or, for an informal association, about how it was set up.
- Print the form, sign it in pen, and file it with form 11 or form 22 for the officers.

Does an informal vereniging have to register?
An association not set up by notarial deed must register if it runs a business. Without a business it may still register voluntarily, which is often useful for opening a bank account.
