What is Formulier 34?
A church organisation reports UBOs only where its registration in the Handelsregister is compulsory. Where it registered voluntarily, the UBO obligation does not follow, and the form says so on its first page.
The UBOs of a church organisation are the natural persons who control it, which in practice means the people at the top of its own governance structure.
Who files Formulier 34?
- Church organisations whose Handelsregister registration is compulsory
- Umbrella bodies reporting UBOs for themselves or their parts
- Organisations updating a UBO after a change in governance
How to fill out Formulier 34
- Confirm that the organisation registration is compulsory rather than voluntary.
- Give the KVK number of the organisation.
- For each UBO give the name, date and place of birth, nationality, address and burgerservicenummer.
- Say on what basis the person is a UBO and from what date, as dd-mm-yyyy.
- Print, sign in pen, and send the form with the supporting documents.
