What is Formulier 30?
A UBO is always a natural person: someone who ultimately owns or controls the company, through shares, voting rights or other means. The UBO register exists to make that ownership traceable, and it sits alongside the Handelsregister rather than inside it.
One copy of the form registers up to four UBOs. Where the company is already in the Handelsregister, KVK asks you to report UBOs online at kvk.nl/ubo instead, which is faster.
Who files Formulier 30?
- A newly incorporated bv or nv registering its UBOs
- Companies where ownership has changed and a new UBO must be registered
- An SE or SCE with its seat in the Netherlands
- Filers who cannot use the online UBO service
How to fill out Formulier 30
- Give the KVK number of the company, or file this with the registration form.
- For each UBO give the name, date and place of birth, nationality, address and burgerservicenummer.
- State the nature and size of the interest held, and the date it started, as dd-mm-yyyy.
- Enclose the documents that evidence the interest, as the form lists them.
- Print, sign in pen, and send the package to KVK.

Frequently asked questions
What if nobody holds more than 25 percent?
The senior managing officials are then registered as pseudo-UBOs. The obligation does not disappear because ownership is spread thinly.
Who can see the UBO register?
Access is restricted rather than fully public, following the Court of Justice ruling on public access. Competent authorities and parties with a legitimate interest can consult it.
