What is Formulier 32?
In a partnership the UBO is usually found in the share of ownership or the share of the profit, or in the ability to control decisions. Each UBO is a natural person, even where the partner itself is a company.
One copy of the form registers up to four UBOs, with the basis and size of each interest.
Who files Formulier 32?
- Newly registered partnerships
- A vof or cv where the partners or profit shares have changed
- An EESV or rederij registering its UBOs
How to fill out Formulier 32
- Give the KVK number of the partnership, or file this with the registration form.
- For each UBO give the name, date and place of birth, nationality, address and burgerservicenummer.
- State the nature and size of the interest and the date from which it applies, as dd-mm-yyyy.
- Enclose the evidence the form lists, such as the partnership agreement.
- Print, sign in pen, and send the package to KVK.
