What is Formulier 8?
A bv, nv, cooperative, mutual, foundation or association is registered as a legal entity in its own right. When it starts actually trading, the business it runs is registered separately, and that is what form 8 does.
The form records the trade name, the activities, the addresses and the start date of the business, and it also covers a legal entity taking over an existing business and continuing it.
Who files Formulier 8?
- A registered bv, nv or cooperative that starts trading
- A stichting or vereniging that begins running a business
- A legal entity taking over an existing business from someone else
How to fill out Formulier 8
- Give the KVK number of the legal entity that will run the business.
- Say whether the business is new or continues an existing one, with the date as dd-mm-yyyy.
- Enter the trade name and describe the activities, products and services.
- Give the visiting address, postal address and contact details of the business.
- Print, sign in pen, and send the form to KVK.

Frequently asked questions
My bv is already registered. Why another form?
The Handelsregister separates the legal entity from the business it runs. Registering the bv does not register its trading activity, and lenders and customers look at the business entry.
