What is N-565?

Your naturalization or citizenship certificate is primary proof of US citizenship, and USCIS does not simply reprint it on request. Form N-565 is the application for a replacement when the original was lost, stolen, mutilated, or destroyed, when your name, date of birth, or gender marker has legally changed, or when USCIS made a typographic error on the original.

The same form requests a special certificate of naturalization for a foreign government, used when another country needs official recognition of your US citizenship. It cannot be used to get a first certificate; that is Form N-600 for citizenship claims or the naturalization process itself.

Who files N-565?

Naturalized citizens whose certificate was lost, stolen, damaged, or destroyed
Citizens whose legal name, date of birth, or gender marker changed after the certificate was issued
People whose certificate contains a USCIS clerical error, which may be corrected without a fee
Citizens who need a special certificate of naturalization for recognition by a foreign government

How to fill out N-565

Identify the certificate you are replacing and the reason: loss, damage, legal change, or error.
Gather supporting documents, such as a court order for a name change or the damaged original certificate.
Include two passport-style photos if the current instructions require them for your filing.
File online through a USCIS account or by mail to the address in the N-565 instructions, with the current fee; check uscis.gov for the fee and any exceptions.
Keep copies of everything; if the original later turns up after a replacement is issued, return it to USCIS.

Can I use my US passport instead of replacing the certificate?

A valid US passport proves citizenship for most purposes, and many people renew a passport instead. But some agencies and foreign governments ask for the certificate itself, and you need one of the two documents to get the other, so replacing a lost certificate is often still worthwhile.

Is there a fee if USCIS made the mistake?

If the certificate contains a typographic error caused by USCIS, the replacement is generally free. For loss, damage, or changes on your side, the standard fee applies; check the current amount at uscis.gov.
