What is CA 0401?
The account you give here is used to pay out the refund of the specific tax you name on the form, and the Belastingdienst keeps using it until you give another one yourself.
It is the paper alternative to changing the account online, which is why it exists at all. Most people can do this in Mijn Belastingdienst in a minute.

How to fill out CA 0401
- Fill in your name, address and burgerservicenummer.
- Give the IBAN of the account, and the name it is held in.
- Say which tax or allowance the account should be used for.
- Sign the form.
- Post it to Belastingdienst, Centrale administratieve processen, Antwoordnummer 21020, 6400 XC Heerlen.

Can the account be in someone else name?
The Belastingdienst pays refunds into an account in your own name. An account in a different name is refused, which is a fraud control rather than an administrative preference.
Does this change my direct debit too?
No. This form is about refunds. A direct debit is arranged separately.

Allow several weeks. If a refund is due sooner, it may still go to the old account.
