KETs Template: Key Employment Terms sample form (blank template)
The KETs template is MOM's ready made layout for issuing Key Employment Terms in writing, published with a filled in sample so employers can see what a completed set looks like.
This is the genuine MOM PDF, unmodified. Source: MOM official page.
What is KETs Template?
Key Employment Terms are the written terms an employer covered by the Employment Act has to give an employee. MOM sets out the items that must be included, covering employer and employee details, job title and main duties, employment start date and contract duration for a fixed term contract, working hours, working days and rest day, salary period and payment dates, basic and gross rates of pay, overtime rate and overtime payment period, fixed allowances and deductions, other salary related components, CPF contributions payable, leave types and medical benefits, probation and notice period.
The template arranges those items over Sections A to E on a single page, followed by a completed sample and guidance notes. It is a template rather than a submission form: you issue it to the employee and keep a copy. MOM also publishes a KETs verification tool, and the template is available in other languages besides English.

Frequently asked questions
Do I have to use this exact template?
No. The template is a convenience. What matters is that the required items are all covered in writing. MOM publishes a KETs verification tool you can use to check a set of terms against the requirements.
Which employees must be issued KETs?
KETs are required for employees covered by the Employment Act, and MOM sets out the coverage and the timing for issuing them. Check the current rules on mom.gov.sg before you rely on an exemption.
Do overtime items always have to be included?
Not always. MOM's guidance says that if the employee is a PME and overtime pay does not apply, the KETs do not need to include the overtime payment period or the overtime rate of pay.
