V112: Declaration of exemption from MOT
Form V112 is the declaration you sign to confirm that a vehicle is exempt from MOT testing, and produce when you tax it at a Post Office.
This is the genuine DVLA PDF, unmodified. Source: DVLA official page.
What is V112?
Evidence of an MOT test is normally required for taxing purposes. Where the vehicle is exempt from MOT testing, DVLA asks you to fill in a V112 and produce it when you tax the vehicle at a Post Office branch. The declaration is short: the registration number, your full name, the letter of the exemption category from the list on the back of the form, and your signature.
The categories on the form cover motor tractors, track-laying vehicles, works trucks, invalid carriages up to 306kg unladen, hackney carriages and private hire vehicles whose roadworthiness the licensing authority checks, trams, vehicles with a maximum design speed under 15.5mph, certain island vehicles, and vehicles registered or manufactured 40 or more years ago that have not been substantially changed in the last 30 years. Goods vehicles over 3,500kg, and other vehicles in type approval categories N2, N3, O3, O4 or T, use form V112G instead.

Frequently asked questions
Which form do I use for a heavy goods vehicle?
V112G, the declaration of exemption from goods vehicle testing, for a goods vehicle over 3,500kg or another vehicle in type approval categories N2, N3, O3, O4 or T. It can also be used for an exempt Northern Ireland goods vehicle of 3,500kg or less.
What does substantially changed mean for a 40 year old vehicle?
The form treats a vehicle as substantially changed where the chassis or monocoque bodyshell, the engine, or the axles and running gear have changed in the last 30 years, with a list of exceptions such as replacements of the same pattern as the original. DVLA publishes fuller guidance on historic and classic vehicle MOT exemption criteria.
Does an exemption mean the vehicle does not need to be roadworthy?
No. Being exempt from testing does not change the requirement to keep a vehicle in a roadworthy condition, and it remains an offence to use an unroadworthy vehicle on the road.
